INTRODUCTION

GRANT PURPOSE

GRANT DETAILS
1. Hearing Loss Type:
- Identify the type(s) of hearing loss your students have been identified with (e.g., conductive, sensorineural, mixed) and the degree (mild/moderate/severe/profound).
2. Teaching Environment Type:
- Describe the teaching environment(s) where the funded resources will be utilized (e.g., mainstream classrooms, special education settings, resource rooms, concentrated programs) and the number of students served.
3. Amount Requested: (Select one)
